About the Role

The LTC Charles S. Kettles VA Medical Center is seeking a dedicated Registered Nurse to serve as a Care Coordinator in our Pulmonary Clinic. In this role, you will guide Veterans and their families through complex health journeys, ensuring they receive the right care at the right time. You will work closely with providers, patients, and caregivers to improve outcomes, enhance quality of life, and make the healthcare system easier to navigate.

Key Responsibilities

  • Coordinate care for Veterans with acute, chronic, and complex conditions, from diagnosis through treatment and follow-up.
  • Serve as a primary point of contact for patients, families, and caregivers, ensuring continuity of care across inpatient, outpatient, and community settings.
  • Collaborate with physicians, specialists, and the broader care team to streamline services and treatment plans.
  • Review and manage lab and imaging results, providing timely updates to providers.
  • Track patient care using electronic systems and monitor outcomes to ensure quality and timeliness.
  • Support patients and families through education, resources, and advocacy for holistic care.

Qualifications

  • Current, unrestricted RN license in any U.S. state or territory.
  • Graduate of an accredited nursing program (ADN, BSN, MSN, or higher).
  • Basic Life Support (BLS) certification required prior to start.
  • Strong communication and organizational skills, with the ability to coordinate across multiple specialties.
  • Compassionate, adaptable, and able to thrive in fast-paced and complex care environments.
