Summary

Incumbent assists RN coordinating care focused on patient transition through continuum of care, patient & family education, patient self management & supporting factors impacting customer satisfaction. Considers characteristics of the individual including age & life stages, state of health, race, culture, values & previous experiences. Administers medications & procedures per established policies & guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team.
