Summary

THIS IS A NATIONAL GUARD TITLE 5 EXCEPTED SERVICE POSITION.

This National Guard position is for a NURSING (CASE MANAGER) - TITLE 5, Position Description Number PDT5637P01 and is part of the KY J1, National Guard.

Duties

As a NURSING (CASE MANAGER) - TITLE 5, GS-0610-11, you will interpret and translate service member (SM) limitations from non-military providers. Draft and submit recommendations for SMs with health limitations to profiling military provider for consideration and signature. Assist in ongoing patient care coordination. Support weekend Medical Readiness Events to schedule meetings with SM in case management, update unit leadership, attend medical synchronization meetings and provide dental support. Maintain mobilization medical readiness information databases and spreadsheets to provide status reports. Provide advice, guidance, and health teaching to SM and Chain of Command to assist in understanding, accepting, and following medical recommendations. Coordinate comprehensive health care services to support individual Medical Readiness requirements in support of State medical readiness. Present cases and summary information to senior leaders and State Surgeon Staff. Conduct reviews with health care team (OSS) and chain of command. Provide support, clinical expertise, and coaching to State Surgeon's Office personnel and Traditional Drilling Medical Providers. Assists in processing of line of duty investigations. May assist the Office of the State Surgeon, Health Systems Specialist staff, and Medical Readiness NCOs awaiting IDES determination and/or return to duty. Ensure Medical board (PEB/MEB) synchronization and submission is complete. Assist in the referral of SM's to MAR2/SRU/RCMC/INCAP. Coordinate and implement health prevention and education programs related to fitness for duty, medical readiness, and deployability. Facilitate and coordinate communication and education between SM and members of the health care team to optimize medical readiness. Provide education and guidance on medical transition programs IAW regulatory and statutory guidance. Assume responsibility and accountability for case management of all Soldiers in case load. Integrate management and case management processes to provide a framework for safe, effective, ethical decision-making within each of the case management team's roles. Perform other duties as assigned.

Must obtain and maintain a favorable adjudication of a T3 or equivalent investigation.
This position is covered by the Domestic Violence Misdemeanor Amendment (30 Sep 96) of the Gun Control Act (Lautenberg Amendment) of 1968. An individual convicted of a qualifying crime of domestic violence may not perform the duties of this position.
Must be able to obtain and maintain credentialing, privileging through the National Guard's Centralized Credentialing and Privileging Preparation Board.
Possess a valid, unrestricted license in the US State, territory, or the District of Columbia to practice medicine or transferable license from other states, board eligible or board certified to include maintaining continuing medical education (CME) hours.
Must be HIPPA trained and maintain certification.
May be subject to drug testing.

Qualifications

MINIMUM REQUIREMENTS:
Must have passed the National Council Licensure Examination.
Must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.

SPECIALIZED EXPERIENCE:
Must have at least two (2) years experience at the next lower level in the normal line of progression through federal service (GS-09) or equivalent experience in the private or public sector that demonstrates the experience and knowledge of the principles and concepts defined by a practical knowledge of health services activities such as, administration, managed care, human resource management, logistics management, patient administration, budgetary and fiscal management, medical manpower, medical facility management, biometrics, medical recruiting, or aeromedical evacuation. Complex knowledge of nursing theory and practice with clinical hands-on experience to evaluate, assess, and recommend the appropriate service member healthcare plans. Experience identifying case management issues, determining medical readiness, and making recommendations for improvement. Knowledge and professional nursing clinical expertise that provides the ability to assess, evaluate and recommend case management interventions. Experience using automated systems to input and analyze data relevant to case management. Experience preparing and conducting briefings, reports, or presentations.

Education

Undergraduate Degree in Health Administration, Health Care Administration (or equivalent), Health Care Management, Health Management and Policy, Health Services Administration (or equivalent), Hospital Administration (or equivalent), or related fields such as, Primary Care Management, and Bachelors of Science in Nursing (BSN/RN), Biology, or microbiology.

Bachelor's degree in nursing (BSN/RN) or Biology.
Other completed degrees are acceptable in Public Health, Entomology, Microbiology, Zoology, Biomedical Engineering, Clinical Engineering and Health Management/Health Systems Engineering, Operations Research, or other closely-related degree fields.

-OR-

Certification as an Occupational Health Nurse or a Graduate Degree in Public Health (MPH/MSPH) with a bachelor's degree in a biological science.

-OR-

Master's degree in Health Administration, Health Care Administration, Health Care Management, Health Management and Policy, Health Services Administration (or equivalent), or Hospital Administration.

-OR-

Master's degree or equivalent in public health, and one year of graduate medical education completed.
