Summary

This is a Direct Hire Open Continuous Announcement (OCA) for Nurse (Clinical Care Coordinator) for the Phoenix Indian Medical Center and will report to the Supervisory Nurse.

The Nurse Clinical Care Coordinator ensures patients at the Phoenix Indian Medical Center receive quality patient centered care that is consistent with the patient-centered medical home principles, and that care is timely, appropriate, cost effective, sensitive and coordinated to affect desired clinical outcomes.
