Duties
The senior social worker in this role provides advanced social work services as a member of the Primary Care Mental Health Integration (PCMHI) team at the Appleton VA CBOC. The incumbent functions as part of a multidisciplinary care team in a primary care setting and delivers brief consultation-based services primarily to Veterans and primary care providers.
- Demonstrates advanced knowledge and clinical skills to operate as a clinical resource with a high degree of independence in providing mental health services.
- Demonstrates an advanced degree of knowledge and fidelity to recovery-based care principles, offering a range of specialized and evidence-based interventions and treatment modalities.
- Offers case consultation and ability to provide supervision within this role.
- Develops, utilizes, and monitors methods for measuring effectiveness of clinical and other social work services, using available outcome evaluation and other objective data to improve care and assist in personal and system improvement efforts.
- Provides generally brief (20-30 Minutes), limited Primary Care Social Work Visits as a routine Primary Care service.
- Assists in the assessment, intervention, and management of mental and behavioral health conditions in the enrolled population.
- Assists as a primary referral source for more acute patients seen in primary or specialty care areas requiring immediate care or brief intervention.
- Provides functional assessment, triage, intervention, education and consultative services.
- Assists in accessing care for Veterans who are in need of psychiatric stabilization.
- Develops treatment plans in collaboration with the veteran/family and with the interdisciplinary treatment team.
- Conducts group therapy groups.
- Provides advanced case management services and demonstrates knowledge of local, state and other VA programs and resources.
- General knowledge of Veteran's benefits and services relating to special programs, service-connected compensation, and nonservice connected pension.
- Facilitates referrals based upon needs and eligibility.
- Independently identifies high-risk patients and implements appropriate safety protocols.
- Provides crisis intervention services and has working knowledge of referral process for inpatient psychiatric hospitalization and residential specialty programs.
- Coordinates family conferences when indicated and serves as liaison to family members.
- Performs other duties as assigned.

Pay: Competitive salary and regular salary increases When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications, or special needs of the VA (Above Minimum Rate of the Grade).
Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
- United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- English Language Proficiency: Social workers must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, chapter 3, section A, paragraph 3j.
- Education: Must have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. (TRANSCRIPTS REQUIRED)
- Licensure: You must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure.
In addition to the basic requirements for employment, the following criteria must be met when determining the grade of candidates.
Grade Determinations:
GS-12 Level: You must possess one year of experience equivalent to the GS-11 grade level (
VA or Non VA) Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency.
Examples of specialized experience at the GS-11 include, but are not limited to:
- assessing and documenting identified behaviors or symptoms of abuse, neglect, exploitation and/or intimate partner violence
- use of clinical social work skills
- maintaining patient privacy and confidentiality per policies, handbooks or directives
- acting as an advocate with appropriate community service providers and agencies when it serves the best interest of the patient and family members/caregiver
- independently assessing the psychosocial functioning and needs of patients and their family members, identifying the patient's strengths, weaknesses, coping skills, and psychosocial acuity.

In addition to the experience above, you must demonstrate the following Knowledge, Skills and Abilities:
- Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management.
- Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice.
- Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes.
- Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills.
- Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area.
