Summary

The Healthcare for Homeless Veteran Outreach Social Worker works with the HCHV Team and focuses on helping homeless Veterans experiencing a wide range of complicated mental, emotional, behavioral, and environmental problems though engagement, assessment, referral and care coordination with the goal of ending Veteran's homelessness.
