Summary
This position serves on the Patient Aligned Care Team (PACT) and Specialty Care. is assigned to North Florida/South Georgia Veterans Health System, located in The Villages, FL. Responsibilities include the assessment, diagnosis, and treatment planning of mental health and psychosocial problems, often when the Veteran is experiencing crisis and include the provision of evidence based clinical/psychotherapeutic interventions, working with Veterans and/or their family members/caregivers.

Duties
This position serves on the Patient Aligned Care Team (PACT) and Specialty Care. The Clinical Social Worker is administratively and professionally responsible to the Chief of Social Work Service or designee. The Clinical Social Worker practices independently, using professional judgment and expertise to make decisions about services and treatment provided to Veterans, family members, and caregivers. The Clinical Social Worker is expected to make independent decisions and requires minimal supervision.
Major Duties include but are not limited to:
- Conducts psychosocial/mental health assessments to identify needs and formulate a patient centered treatment plan with the Veteran to meet needs and goals.
- Responsible for the development of patient centered treatment plans and developing achievable treatment goals with the Veteran and other significant others.
- Provides case management and care coordination to facilitate appropriate delivery of health care services,
incorporating complex multiple causation in differential diagnosis and treatment of Veterans and making psychosocial and/or mental health diagnoses
- Provides crisis intervention services, seeking to address the cause as well as the presenting complaint, coordinates family conferences and serves as liaison to family members.
- Provides individual and group psychotherapy using evidence based modalities
- Responsible for planning and organizing work, coordinating with clinical interdisciplinary team members, other professional and clinical staff, and management personnel.
- Enters all Veteran/family interactions in the electronic heath record, using appropriate formats and templates within the designated timeframe, according to policy. Properly codes all work to include identifying
encounterable work and utilizing appropriate diagnosis and procedure codes.
- Proactively addresses advance directive education and facilitates completion of advance directives,
- Serves as the subject matter expert on VA and community resources.
- Manages virtual care appointments through VA approved modalities/technology, when the Veteran is unable to present for an appointment in person.
- Provides patient education to Veterans and family members on the available services and assists them in establishing the appropriate referrals based on the Veteran's preference or other decision maker
- Responsible for developing, updating and using current social worker resources within VA and community social service programs to ensure that Veterans are receiving education, information and referrals to current and viable resources to meet their needs.
- Develops and executes crisis management treatment plans in a timely, effective and efficient manner.
Preferred experience:
- Mental Health experience and/or Health Care experience.
- Case Management experience.

Additional Benefits:
Pay: Competitive salary and regular salary increases. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of existing pay, higher or unique qualifications, or special needs of the VA (Above Minimum Rate of the Grade).
Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year). Selected applicants may qualify for credit toward annual leave accrual, based on prior work experience or military service experience.
Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.
Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66 .

Basic Requirements:
- United States Citizenship: Must be US Citizen. Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. ยง 7403(f).
- Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may NOT be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work.
- Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. The incumbent must be licensed or certified by a state at the independent or advanced practice level as determined by an ASWB advanced generalist or clinical examination. - May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations: In addition to meeting the basic requirements stated above, the following qualifications criteria must be met in determining the appropriate grade assignment of candidates.
GS-11 Social Worker candidates must have the following:
- Experience and Licensure: Completion of a minimum of one (1) year of post-MSW experience equivalent to the GS-9 grade level in the field of health care or other social work-related settings, (VA or non-VA experience) and licensure or certification in a state at the independent practice level.
OR
- Education: In addition to meeting basic requirements listed above, a doctoral degree in social work from a school of social work may be substituted for the required one (1) year of professional social work experience in a clinical setting. You must attach additional transcripts to support this qualification.

- Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services.
- Skill in independently conducting psychosocial assessments and treatment interventions to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diversified backgrounds.
- Knowledge of medical and mental health diagnoses, disabilities and treatment procedures (i.e. acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology) to formulate a treatment plan.
- Skill in independently implementing different treatment modalities in working with individuals, families, and groups who are experiencing a variety of psychiatric, medical, and social problems to achieve treatment goals.
- Ability to provide consultation services to new social workers, social work graduate students, and other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment.

The full performance level of this position is the GS-11.
Physical Requirements: The physical demands of the work requires use of fingers; moderate walking and standing; near vision correctable at 13" to 16" to Jaeger 1 to 4; far vision correctable in on eye to 20/20 and to 20/40 in the other; hearing aid permitted; clear speech; emotional stability; moderate duty. Ability to drive/operate government vehicle if needed. The work requires patience and control of emotions. The work may require occasional use of appropriate techniques to physically restrain clients who present a danger to self or others.
