Duties
As the Coordinator, the incumbent assists in planning, establishing, and implementing policies and procedures; developing program goals and objectives as well as monitoring, operating, evaluating, coordinating and daily operations of the assigned programs to ensure Veteran access to purchased home and community service. Responsibilities include liaison with community providers, identification of community resources, staff education on program services, referral processes, and the development, evaluation and communication of standards, policies and proceed.
Major duties includes, but aren't limited to:
Veterans Directed Care:
- Provides administrative, technical, budgetary monitoring and oversight necessary for accomplishing the program coordination and management functions for the VDC program.
- Provides Technical assistance provided to monitor activities and program reviews, and the development and analysis of information.
- Initiates consultation with the GEC SW Program Coordinator and/or the Chief or Deputy Chief of Social Work Services, as well as, with other authoritative resources at the Medical Center, such as Regional Counsel, and Fiscal Services in unusual and/or complex administrative and clinical problems.
- Develops and implements information systems to track service activities including visits, referrals, patient demographics, and patient satisfaction.
- Schedules, organizes, and inspects community provider facilities of each program respectively and the completion of appropriate required reports and follow-up to ensure program operation.
- Negotiates, facilitates, and completes basic ordering agreements/contracts for the VDC.
Adult Day Health Care:
- Receives all approved ADHC consults for continued management of referrals.
- Coordinates placement between the Veteran, family, and Adult Day Healthcare (ADHC) Center.
- Provides weekly, monthly, and/or quarterly reports for Veteran enrolled, discharged, or seeking ADHC services.
- Serves as the liaison with ADHC providers to resolve clinical concerns in collaboration with the Veteran's primary care and GEC community teams.
- Performs work related to the review, monitoring, analysis, planning, and assessment of the quality of care provided to Veterans services.
- Offers consultation to colleagues and students on the psychosocial treatment of patients treated in specialty areas, rendering professional opinions based on experience and expertise and role modeling effective social work practice skills.
- Provides written response to Congressional, White House Line and Patient Advocacy inquiries/complaints, performs conflict resolution, and service recovery for all personal Care Services.

Qualifications
Basic Requirements:
- Citizenship. United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going tohttp://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work.
- Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level.
- English Language Proficiency. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f).
Differentiating Independent Practice from Advanced Practice Level. Experience must be evaluated to differentiate between independent practice and advanced practice levels. A social worker practicing at the independent level has a generalized knowledge of practice, (which is required for all social workers at the full performance level and above.), whereas the social worker practicing at the advanced level has specialized knowledge of practice typically related to a particular diagnosis or patient population. The advanced practice social worker can be further differentiated from the independent practice social worker by their ability to expand clinical knowledge in the profession, provide consultation and guidance to colleagues, role model effective social work practice skills, teach or provide orientation to less experienced social workers, develop innovations in practice interventions, and provide clinical supervision for social work licensure or certification. A social worker with advanced practice skills and personal qualifications that meet the standard for the senior social worker will have passed an advanced generalist or clinical Association of Social Work Boards (ASWB) examination and be assigned to a position that requires use of the advanced practice skills in order to be considered for the senior social worker grade.
Grade Determinations:
Social Worker (Program Coordinator), GS-12
(1) Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
(2) Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure.
(3) Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs:
(a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring.
(b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program.
(c) Ability to supervise multidisciplinary staff assigned to the program.
(d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
(e) Ability to provide training, orientation, and guidance within clinical practice.
(4) Assignments. For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and variety and be performed by the incumbent at least 25% of the time. Program coordinators at the GS-12 grade level are administratively responsible for a clinical program providing treatment to Veterans in a major specialty area, such as but not limited to: Spinal Cord Injury, Homeless Continuum Veteran Program, Hospice and Palliative Care Program, Suicide Prevention Program, Veterans Justice Outreach, Caregiver Support Program and Community Nursing Home Program. The program coordinator may be the sole practitioner in this specialty at the facility and typically provide direct patient care services in the program area. The program coordinator oversees the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. They may be responsible for the program's budget, developing and monitoring staff compliance with practice, standards and guidelines on documentation, workload, data entry, ethical practice and service delivery. The program coordinator provides analysis and evaluation of clinical program data and computerized programs to identify system-wide trends and needs to enhance the quality of service. They may be responsible for, or contribute to, the program's resource and fiscal management, monitoring control points developing the annual budget, operating within that budget, and accounting for appropriated funds. The program coordinator is administratively responsible for the clinical programming and prepares reports and statistics for facility, VISN, and national use. They provide 14 leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for assigned staff. They initiate and conduct a variety of program or service audits and complete designated clinical practice audits and reports, including productivity assessments. They oversee program operations and evaluations, identifying areas for improvement, gathering relevant data, assessing the data, developing and implementing ideas for improvement and evaluating efficacy of improvement efforts.

The full performance level of this vacancy is GS-12.
Physical Requirements: The physical demands of this position involve active listening, reading, observing and evaluating verbal and nonverbal behavior, walking, speaking, bending, carrying light items such as paper, books, small parts, writing reports, writing clinical notes, using office equipment such as computers, telephones, fax machines, photocopy machines, and possibly driving an automobile. Some work is performed in a clinical setting that exposes the incumbent to the communicable disease common of the homeless population. Strenuous physical activity is not required.
